Social Prescribing Link Worker

What is Social Prescribing?

The aim of social prescribing to help peoples' mental, physical and social health by linking to local non clinical services primarily provided by the community or voluntary services.  You can self refer into the service or be referred by a health professional such as a GP or public health nurse.

Who needs Social Prescribing?

If you are over 18 and are experiencing loneliness, low mood, social isolation, physical health issues or family issues, social prescribing may be a way to address your needs.

You will meet Michelle Foley, our Social Prescriber who will have a series of conversation with you to discuss how to create a tailored approach to help you improve your physical and mental wellbeing. This will be done by improving social connectedness.
